NHỮNG KẾT QUẢ MỚI CỦA LUẬN ÁN

1. Luận án là một trong những nghiên cứu thuần tập đầu tiên tại Việt Nam theo dõi liên tục tăng trưởng thể chất của trẻ sinh đủ tháng từ lúc sinh đến 10 tuổi, đồng thời đánh giá nhận thức và năng lực học tập lúc 10 tuổi. Nghiên cứu cung cấp bộ số liệu theo dõi dọc trên 703 trẻ tại Thái Nguyên, góp phần bổ sung bằng chứng về tăng trưởng và phát triển của trẻ em Việt Nam.

2. Nghiên cứu mô tả toàn diện sự thay đổi về chiều dài/chiều cao, cân nặng, chu vi vòng đầu và tốc độ tăng trưởng qua các giai đoạn 0 - 2 tuổi, 2 - 6 tuổi và 6 - 10 tuổi. Tăng trưởng của trẻ tuân theo quy luật chung, đã cải thiện rõ so với trẻ em Việt Nam thập niên 1990 nhưng vẫn thấp hơn chuẩn tăng trưởng của Tổ chức Y tế Thế giới và một số quần thể quốc tế.

3. Luận án xác định được sự biến đổi của tình trạng dinh dưỡng và quỹ đạo tăng trưởng trong 10 năm đầu đời. Theo chỉ số chiều cao theo tuổi, tỷ lệ trẻ phát triển bình thường tăng từ 73,8% lên 90,8%, tỷ lệ tăng trưởng chậm giảm từ 17,5% xuống 0,4%; đồng thời nhận diện các nhóm bắt kịp tăng trưởng, tăng trưởng chậm và suy dinh dưỡng kéo dài theo từng giai đoạn.

4. Nghiên cứu cung cấp dữ liệu về nhận thức của trẻ 10 tuổi tại Thái Nguyên bằng thang đo WISC-IV. Điểm trí tuệ tổng hợp trung bình là 90,99 ± 12,86; trẻ nữ có điểm tư duy tri giác và tốc độ xử lý cao hơn trẻ nam, còn trẻ dân tộc Kinh có điểm trí tuệ tổng hợp và kết quả học tập cao hơn trẻ dân tộc khác.

5. Luận án chứng minh tăng trưởng thể chất có mối liên quan dài hạn với nhận thức và năng lực học tập. Chỉ số chiều cao theo tuổi có mối liên quan ổn định nhất; chiều cao theo tuổi và chu vi vòng đầu ở 2 tuổi liên quan thuận với điểm trí tuệ tổng hợp lúc 10 tuổi, trong khi suy dinh dưỡng thấp còi ở 2 tuổi và 6 tuổi liên quan với điểm Toán và Tiếng Việt thấp hơn.

6. Phân tích quỹ đạo tăng trưởng cho thấy trẻ tăng trưởng chậm trong giai đoạn 0 - 2 tuổi và trẻ suy dinh dưỡng thấp còi kéo dài có điểm trí tuệ tổng hợp thấp hơn nhóm phát triển bình thường. Trẻ bắt kịp tăng trưởng có điểm trí tuệ tổng hợp tương đương nhóm phát triển bình thường, gợi ý rằng phục hồi tăng trưởng sớm có thể góp phần hạn chế ảnh hưởng bất lợi của suy dinh dưỡng đối với nhận thức.

Kết quả nghiên cứu có thể được sử dụng làm cơ sở khoa học cho việc xây dựng và củng cố chương trình theo dõi tăng trưởng thể chất của trẻ từ lúc sinh đến 10 tuổi, đặc biệt chú trọng chiều cao theo tuổi, cân nặng theo tuổi và quỹ đạo tăng trưởng. Việc phát hiện sớm tăng trưởng chậm, suy dinh dưỡng thấp còi, nhẹ cân và suy dinh dưỡng kéo dài giúp triển khai can thiệp dinh dưỡng, chăm sóc sức khỏe và hỗ trợ phát triển phù hợp; đồng thời cần lưu ý sự khác biệt theo giới và dân tộc.

Có thể kết hợp giám sát nhân trắc với sàng lọc nhận thức và năng lực học tập ở trẻ có nguy cơ nhằm phát hiện sớm và hỗ trợ kịp thời. Cần tiếp tục thực hiện các nghiên cứu đa trung tâm với cỡ mẫu lớn hơn, phạm vi đại diện rộng hơn và thời gian theo dõi dài hơn; đồng thời áp dụng các mô hình phân tích dọc nâng cao để làm rõ quỹ đạo tăng trưởng, các yếu tố trung gian và cơ chế tác động của tăng trưởng thể chất đến nhận thức.

NEW CONTRIBUTIONS OF THE DISSERTATION

1. This dissertation is among the first cohort studies in Vietnam to continuously follow the physical growth of full-term children from birth to 10 years of age, while also assessing their cognitive function and academic performance at 10 years of age. The study provides longitudinal data on 703 children in Thai Nguyen Province, thereby contributing additional evidence on the growth and development of Vietnamese children.

2. The study comprehensively describes changes in length/height, weight, head circumference, and growth velocity across the age periods of 0–2, 2–6, and 6–10 years old. Children’s growth followed the expected general pattern and showed marked improvement compared with that of Vietnamese children in the 1990s; however, it remained below the World Health Organization growth standards and those reported for several international populations

3. The dissertation characterizes changes in nutritional status and growth trajectories during the first 10 years of life. Based on height-for-age status, the proportion of children with normal growth increased from 73.8% to 90.8%, while the proportion with growth faltering decreased from 17.5% to 0.4%. The study also identified groups of children with catch-up growth, growth faltering, and persistent undernutrition across different developmental periods.

4. The study provides data on cognitive function among 10-year-old children in Thai Nguyen Province, assessed using the Wechsler Intelligence Scale for Children—Fourth Edition (WISC-IV). The mean Full-Scale Intelligence Quotient (FSIQ) score was 90.99 ± 12.86. Girls had higher Perceptual Reasoning Index and Processing Speed Index scores than boys, while Kinh children had higher FSIQ scores and better academic performance than children from other ethnic groups.

5. The dissertation demonstrates that physical growth is associated with long-term cognitive function and academic performance. Height-for-age showed the most consistent association. Height-for-age and head circumference at 2 years of age were positively associated with Full-Scale Intelligence Quotient scores at 10 years of age, whereas stunting at 2 and 6 years of age was associated with lower Mathematics and Vietnamese language scores.

6. Growth trajectory analysis showed that children who experienced growth faltering during the first 2 years of life and those with persistent stunting had lower Full-Scale Intelligence Quotient scores than children with normal growth. Children who achieved catch-up growth had Full-Scale Intelligence Quotient scores comparable to those of the normal-growth group, suggesting that early growth recovery may help mitigate the adverse effects of undernutrition on cognitive function.

 

PRACTICAL APPLICATIONS, POTENTIAL FOR IMPLEMENTATION, AND ISSUES REQUIRING FURTHER RESEARCH

The study findings may serve as a scientific basis for developing and strengthening programs for monitoring children’s physical growth from birth to 10 years of age, with particular attention to height-for-age, weight-for-age, and growth trajectories. Early detection of growth faltering, stunting, underweight, and persistent undernutrition may support the implementation of appropriate nutritional interventions, healthcare, and developmental support. Differences by sex and ethnicity should also be taken into account.

Anthropometric monitoring can be combined with screening for cognitive function and academic performance in at-risk children to enable early detection and timely support. Further multicenter studies with larger sample sizes, broader representativeness, and longer follow-up durations are needed. In addition, advanced longitudinal analytical models should be applied to clarify growth trajectories, mediating factors, and the mechanisms underlying the effects of physical growth on cognitive function.
